Letter to Prospective Donors
Donation Pledge
Tax Credit Fact Sheet - before giving this to donors be aware that this is an MFA document, and it states the minimum donation amount is $200.  Please explain to your donor that Habitat has voluntarily raised the minimum donation to $1,000 because of the paperwork requirements for this program.

Donation Forms
Tax Credit Donation Deposit Slip - optional, use this to document receipt and deposit of tax credit donations.
Tax Credit Voucher Certificate  - please make sure your donor fills out both pages.
Affidavit of Services  - to document the value of donated professional services

Project Costs & Sources - use this form to demonstrate financing commitments

Design Standards Comparison - compare HFHI's standards with MFA's.
Design Standards Checklist
Example of a good site plan  - this is considered a good  site plan because everything is labeled clearly: rooms, square footage (total and by room), closets, address and homeowner's name.  The ideal site plan should detail all of the information listed in MFA design standards and be easy to read.  

Land Use Restriction Agreements (LURA)

Affiliate LURA (between MFA and affiliate) - MFA staff will draft this document and send to the affiliate for signing and recording.
Homeowner LURA  (between MFA and homeowner) - leave exhibit B blank
